Only you, or someone legally authorized to act on your behalf, may make a verifiable consumer request related to your personal information. You may also make a verifiable consumer request on behalf of your minor child. To designate an authorized agent, you must mail to the above address a notarized power-of-attorney, identifying yourself in a verifiable manner, and designating with specificity the actions that your agent is authorized to perform, or information authorized to request, on your behalf.  The power-of-attorney must be signed by you within 12 months of the date on which we receive the request.
